Verification Process 2010 - 2011
The Office of Financial Aid will notify you if you have been selected for review in the process called Verification. What is Verification and why was I selected? Approximately one-third of all student aid applications are selected for verification by the Department of Education.
To complete the process, provide the following documentation to the Office of Financial Aid:
- Signed and dated Verification Worksheet with all sections completed.
- Signed and dated copies of 2009 US tax returns, W2’s, and all applicable schedules for:
- yourself
- your parents, if you are a dependent student
- your spouse, if you were married at the time of filing your FAFSA.

- If you or one of the relevant persons in number 2 above will not be filing a 2009 US tax return, you or that person may be asked to submit a copy of a Free Tax Transcript 2009 (see directions below) to the Office of Financial Aid in August. NOTE: The Office of Financial Aid may request additional information regarding your support in 2009. If required by the Office of Financial Aid, please complete the Resource Form.
To request a free tax transcript (relevant information printed directly from IRS website):
Request a Free Tax Transcript by telephone or by mail. You can obtain a free transcript by calling 800-829-1040 and following the prompts in the recorded message or by completing and mailing a request for a transcript to the address listed in the instructions. The IRS has created a new Form 4506T-EZ, Short Form Request for Individual Tax Return Transcript, to order a transcript of a Form 1040 series return. Forms can be downloaded by using the following links or ordered by calling 800-829-3676.

- Form 4506T-EZ, Short Form Request for Individual Tax Return Transcript
- A tax return transcript shows most line items from the tax return (Form 1040, 1040A or 1040EZ) as it was originally filed, including any accompanying forms and schedules. It does not reflect any changes you, your representative or the IRS made after the return was filed. In many cases, a return transcript will meet the requirements of lending institutions such as those offering mortgages and student loans. You should receive your tax return transcript within 10 working days from the time the IRS receives your request.
- A tax account transcript shows any later adjustments either you or the IRS made after the tax return was filed. This transcript shows basic data, including marital status, type of return filed, adjusted gross income and taxable income. The IRS does not charge a fee for transcripts, which are available for the current and past three years. Allow 30 calendar days for delivery of a tax account transcript.
